Should we have consistent codebase structuring for all app sizes?

Recently, I was reading an article and bumped into this question in the comments. Does structuring code depend on apps size? Or should it be consistent?

What happens when a small or medium-sized app becomes large over the period? In that case, it would be good to follow proper code structuring from the beginning.

What are your thoughts on this?
